MSHSAA Pre-Participation Form
Only the first two pages are required at the time of your appointment. Your school will require the other pages and possibly more. Be sure to check with your school to ensure you have all of the required paperwork completed.
DOT Physical Form
Please bring this form, driver's license, previous medical certificate, if possible, and any necessary medical documentation with you to the appointment. Please call the office with any questions.
